4-H Enrollment

Congratulations on starting another year or beginning your first year in the Dodge County 4-H Program. Keep in mind that if there is anything you or your 4-H club needs during the 4-H year, don't hesitate to contact us at the Dodge County Extension Office, by email or by phone (402/727-2775) for assistance.

Organizational Leader (or Independent Member) Responsibility

It is the responsibility of the 4-H club's organizational leader (or independent member) to obtain the enrollment/re-enrollment forms and fees from the entire club and submit them to the Dodge County Extension Office. If necessary, each family may submit forms and fees separately from their 4-H club. When enrolling your club, please list names of all volunteers planning to assist with your 4-H club. These volunteers include project leaders (indicate project areas), activity leaders and resource leaders. All volunteers should fill out a 4-H Volunteer Enrollment Form to be turned into the Dodge County Extension Office and complete the "Youth Protection - 4-H Volunteer Screening Form" and submit this form to the Nebraska State 4-H Office.
